How Our Membership Plans Work
Our in-office membership plans are designed for patients without traditional dental insurance.
Each plan includes preventive care and offers savings on additional treatment completed at our office.
Plan eligibility (Adult vs Perio) is determined based on your clinical findings at your comprehensive exam.
Includes:
Two routine cleanings per year
Two comprehensive exams per year
All necessary x-rays associated with exams
Two fluoride treatments
Two oral cancer screenings
Two additional emergency exams per year (if needed)
15% savings on fillings, crowns, extractions, and sealants
Annual Investment: $350
Adult Plan
Includes:
Two routine cleanings per year
Two comprehensive exams per year
All necessary x-rays associated with exams
Two fluoride treatments
Two oral cancer screenings

Annual Investment: $250
Child Plan
(Under 18 years old)
Includes:
Four periodontal maintenance cleanings per year
Four periodontal exams per year
Fluoride Treatments per year
All necessary x-rays associated with exams
Four oral cancer screenings
Two additional emergency exams per year (if needed)
15% savings on fillings, crowns, root canals, extractions, and sealants
Annual Investment: $500
Periodontal Maintenance Plan
How Plan Eligibility Is Determined
Your membership category is based on your gum health and periodontal diagnosis.
During your comprehensive new patient exam, we assess your overall oral health — including your gum condition — to determine whether you qualify for the Adult Plan or the Periodontal Maintenance Plan.
Patients diagnosed with periodontal disease require specialized maintenance and are enrolled in the Periodontal Maintenance Plan to support long-term gum health.
Plan placement is based solely on clinical findings and designed to ensure you receive the appropriate level of care.

Membership fees are due in full at time of enrollment.
Membership benefits are valid for 12 months from enrollment date.
Membership is non-transferable and non-refundable.
Savings apply only to treatment completed at Pampered Smiles Dentistry.
Membership cannot be combined with dental insurance.
